Background:We assessed the efficacy of Arimidex (Anastrozole) 1mg in combination with tamoxifen in postmenopausal women with metastatic breast cancer.
Methods:We conducted a prospective, randomized, double-blind, placebo-controlled study comparing tamoxifen plus Arimidex (3 mg, 5 mg, 7.5 mg) in combination with Arimidex 1mg in postmenopausal women with metastatic breast cancer who had a disease response. The primary end point was the proportion of patients with a disease response to the first and the second treatment (the primary end point). The secondary end points included overall survival (OS), progression-free survival (PFS), distant involvement, disease-free survival (DFS), and the need for surgery.
Results:The results showed that there was no significant difference in the proportion of patients with a disease response to Arimidex (p = 0.63) in combination with tamoxifen (p = 0.65) in the primary end point. However, the patients who took Arimidex plus tamoxifen were more likely to have a disease response (p = 0.003) to surgery. The PFS rate of the patients with a disease response to the first and the second treatment was significantly higher in the combined group (7.8 and 4.5 days, respectively) than in the Arimidex group (2.1 and 2.3 days, respectively). There was no significant difference in DFS between the combined and Arimidex groups (p = 0.84).
Conclusions:In the combined group, the combination of Arimidex and tamoxifen showed superior efficacy in the reduction of disease-free survival and OS compared with Arimidex alone.

There are reports of postmenopausal women receiving Arimidex with tamoxifen for adjuvant treatment of breast cancer, but there have been no reports of patients who are treated with Arimidex with tamoxifen. There is an increased risk of postmenopausal breast cancer recurrence in postmenopausal women with tamoxifen therapy. Arimidex 1mg is contraindicated in women with a positive mammogram. Patients with a positive mammogram should undergo adjuvant treatment with tamoxifen 1mg. The risk of treatment failure is increased in women receiving Arimidex with tamoxifen, but there have been no reports of postmenopausal women receiving Arimidex with tamoxifen.
